If you are scheduled to have surgery at NSMC Salem Hospital or NSMC Union Hospital, we ask that you
submit your medical history online at the secure One Medical Passport web site.

Admitting Packet
Patients admitted to the hospital are given an admitting packet. The packet contains the Patient Bill of Rights, our No Smoking regulations, and information on what will be in your room (TV, etc.). It will also explain your HIPPA rights, including an option to restrict all mail, phone calls and visitors if you prefer complete privacy during your stay. Information about how to designate a Health Care Proxy will be in packet as well.
