Obtaining Your Own Medical RecordsIf you have received treatment at North Shore Medical Center and would like to obtain a copy of your medical records, please fill out one of the forms below.

NOTE: Faxes are NOT ACCEPTED.
Please also be aware that a fee may be associated with your request. Most requests are usually processed within five to ten business days.
Copies of Protected Health Information will not be given unless you provide one of the following forms of identification (expired identification will not be accepted).
- Valid U.S. Driver’s License; or
- Valid U.S. State-issued non-driver identity card; or
- Valid U.S. Passport
If the patient is a minor or is not competent to give consent, the signature of a parent, guardian or other legal representative is required. For deceased patients, additional documentation will be required for release of protected/privileged health information. Contact the Release of Information at the numbers listed below with specific questions.
