Case Managers
If you are hospitalized at either NSMC Salem or NSMC Union hospitals, you will be assigned a case manager. The role of the case manager is to review patients’ hospital records, and meet with those who need assistance during or after their stay. You will meet your case manager if it becomes clear that you might require medical assistance at home once you leave the hospital, or if your physician determines that you will need to transfer to a hospice or nursing home. You will be provided with a list of choices. Placement is determined by your personal preference but does depend also on which program can accept you.
Social Workers and other Resources for Patients
Sometimes your nurse, physician or case manager will refer you to a social worker. You may also request the services of a social worker. Social workers support you in a variety of ways, including helping you fill out important applications and forms (such as a request for financial assistance), and informing you of our many support and counseling services.
