All surgical patients will now be able to complete the medical history portion of their registration online, at their own convenience. 

If you are scheduled to have surgery at NSMC Salem Hospital or NSMC Union Hospital, we will encourage you to submit your medical history online at the secure One Medical Passport web site.

Pre-Surgery Assessment
